Step 1:  All patients must have a  qualifying condition: Severe chronic pain, nausea, seizures, or muscle spasms; Cancer, Glaucoma, HIV/AIDS, Hepatitis C, Crohn's Disease, ALS, agitation of Alzheimer's disease, Epilepsy, Multiple Sclerosis, Nail Patella, Cachexia (wasting syndrome) or PTSD.

Step 2: Patients must obtain and provide medical records about condition. A diagnosis from your doctor is needed if you suffer from Cancer, HIV/AIDS, Glaucoma, or ALS. For all other conditions, we require records of 3 VISITS, within the last 3 YEARS about your qualifying condition.  

Obtaining the records yourself is usually fastest, but we can request your medical records with your signed medical release form and the doctor's FAX and PHONE number.

Please note, we do not offer primary care to manage your other medical needs.

Appointment Fees are $200 paid at time of service. Cash, Money Order, Debit and Credit Accepted. 

Saturdays and Sundays-Charity care and telephone consult available. Personal visits after initial telephone consults.
